Take a new file of 400 pixels, 400 pixels, of resolution 72 dpi in the RGB mode.
Fill the Background with any color would your like, then take the Rounded Rectangle Tool use the following settings.
Create a shape make the selection, then take the Rectangular Marquee Tool use these settings.
Now create a shape over the previous shape as shown image below.
