Step 1:

Let’s start out by creating a new file. I used a 500×500 pixels canvas set at 72dpi, and I filled my background with white color. Now make a new layer set and name it ‘Cold Glass Text’.

Select the Horizontal Type Tool then set the font family to Britannic Bold, regular, 110 pt, strong and #727272 color shade. In a new text layer type your name.

Step 2:

Under Layer Style(Layer > Layer Style) add a Drop Shadow, Inner Shadow, Outer Glow and Inner Glow blending options to your gray text layer.

Step 3:

In a new text layer type your name in white.

Step 4:

Under Layer Style(Layer > Layer Style) add an Inner Glow and Gradient Overlay blending options to your white text layer. Then set the layer’s blending mode to Darken at 63% opacity level.

Step 5:

Now merge your ‘Cold Glass Text’ layer set through Layer > Merge Layer Set. Then duplicate the text design and put it behind the first text design on the layers window.

Under Layer Style(Layer > Layer Style) add an Inner Glow blending option.

Step 6:

Duplicate your text design layer and apply a Plastic Wrap effect through Filter > Artistic, use the settings below.
